Note: If you have already submitted an application to graduate through the York University portal, you do not need to submit a second application on this form. The form below is only to be used if you intend to graduate with a Certificate or Graduate Diploma.

Instructions:
1. Complete this form.
2. After submission, a copy will be sent to you via e-mail. Save a copy for your records.
