This form is intended for returning Schulich master level students with a financial block that prevent the students from enrolling in courses for the current or upcoming academic session. This form must be submitted on or before the last date to add with permission. Please refer to Schulich Important Dates. Requests submitted after this deadline will not be considered. Schulich PhD students should complete the DRA on the FGS website. Schulich BBA/iBBA students should complete the DRA on the Registrar Office website. Please note: -Debt Repayment Plans will only be reviewed if this form is complete. -All supporting documentation must be uploaded under Supporting Files, including a repayment plan outlining how and when you will pay the remaining balance, bank statements, credit card statements, loan applications (e.g. OSAP or student line-of-credit, etc.). -You will be notified by email with a decision once the application has been reviewed. -You must complete all fields applicable to your situation. -It may take up to 10 business days to review the application and render a decision. -For questions, please contact Director, Student Records & Financial Aid via email at studentservices@schulich.yorku.ca.
